If you are scheduled for an appointment at St. John’s Sports Medicine, there are some things you can do to help ensure your visit is as productive as possible. Please take a couple of minutes to review the following information.

For appointments with a Certified Athletic Trainer, please bring:

¢ Your current insurance card (primary and secondary insurance if applicable),

¢ Any prescription medication you are currently taking.

¢ A T-shirt if you have an upper extremity injury or a  pair of shorts if you have a lower extremity injury.

¢ Your prescription for treatment and rehabilitation.

¢ If your visit is being covered by workers' compensation, please provide the name of the person authorizing treatment, company name, address phone number and fax number.

We encourage all patients to call their insurance company to verify payment for the services of a certified athletic trainer prior to coming in for the first visit.   

If patient is a minor, a parent or guardian must be present for the initial visit.  For follow up visits or treatments, minors may be seen without a parent or guardian present if a note or letter of permission has been provided. 

For appointments with a physician, please bring:

¢ Your current insurance card (primary and secondary insurance if applicable).

¢ Any prescription medication you are currently taking.

¢ Current diagnostic tests that pertain to your injury, including X-rays, MRI, CT Scan, bone scan, etc.

¢ Your driver's license or other form of picture ID.

¢ If your visit is being covered by workers' compensation, please provide the name of the person authorizing treatment, company name, address, phone number and fax number.
