AMGA Recognizes Hammons Heart Institute
and News Conference to Preview Addition
of Cardiac Cath Units

A news conference held at 10 a.m., Thursday, Jan. 31, in the new addition of cardiac catheterization units on 3 Northeast at St. John's Regional Health Center, offered a preview of the two new state-of-the art cardiac cath units bringing the number to six, which will provide better patient care.
An announcement was also made about the American Medical Group Association's selection of St. John's Hammons Heart Institute program as an honoree for the national Acclaim Award. One winner and four honorees from across the nation were named to receive Acclaim Awards during the AMGA national conference in San Diego on Feb. 22.
Representatives from St. John's Health System will make a presentation about the program entitled Tele-management Improves Outcomes in Congestive Health Failure Patients, during the conference and accept an award of $10,000 for an educational grant.
Acclaim Awards are given in recognition of clinics and health care systems for initiatives and programs that demonstrate dedication to excellence and quality in health Care. St. John's Hammons Heart submission was chosen as an honoree due to positive patient outcome results, which recorded marked improvements for patients with congestive heart disease.
Acclaim Award Information
- The Acclaim Award is an annual national award presented by the American Medical Group Association (AMGA), which is an organization comprised of large multi-specialty clinics. The AMGA is part of the Medical Group Management Association.
- The award recognizes clinics and health care systems for initiatives and programs that demonstrate a dedication to excellence and quality in health care.
- St. John's Hammons Heart Institute submitted an application entitled "Tele-Management Improves Outcomes in Congestive Heart Failure Patients" and received one of three "Honoree" Awards.
- Awards were also presented to Park-Nicollet Health Services, Lake City Medical/Mayo Clinic, Kaiser Permanente/Georgia, and Aurora Health Care.
- St. John's Hammons Heart Institute implemented the "Heart Failure Management Program" in 1996 and currently there are 1,229 patients active in the program. The goal of the program is to improve outcomes for patients with congestive heart failure. The program consists of three key components:
- Coordination of all facets of inpatient care coupled with an intensive patient education program on congestive heart failure.
- An intensive post-hospitalization tele-management program whereby all patients are linked to care givers through regularly scheduled telephone calls.
- A specialized exercise program designed for CHF patients.
- St. John's Hammons Heart Institute "Heart Failure Management Program" has had several noteworthy outcomes. Key outcomes include:
- Admission rates for congestive heart failure have decreased by 17.5% since program inception.
- Re-admission rates for patients with a primary diagnosis of congestive heart failure (CHF) have been reduced. The re-admission rate for re-admissions occurring within 30 days of discharge has been reduced from 7.6% in 1997-98 to 4.0% in 1999-2000. The re-admission rate for re-admissions occurring within 180 days of initial discharge has been reduced from 20.8% to 14.0%.
- Quality of life for patients was improved as evidenced by improved scores on the "SF-36 Health Status Profile" which was completed before and after the program.
The overall physical health summary improved from a score of 32 to a score of 35 and the mental health summary improved from 45 to 48.
- Patients improved dramatically in their ability to exercise, which also impacts quality of life. The average walk distance increased from 1307 feet to 1755 feet, a relative improvement of 34.2%.
