St. John's Heart Failure Resource Center is a
support program for patients who have been diagnosed with Heart Failure (HF).
The goal of the program is to help HF patients stay in control of their
health by teaching them about HF symptoms and treatment, and by helping them
to manage the disease process.
The program consists of:

Telemonitoring of patient symptoms - Patients enrolled in the
program are phoned on a regular basis by specially trained cardiac nurses.
By monitoring patient symptoms over the phone, our cardiac nurses can
provide early intervention if symptoms worsen.

Patient education - Patients receive ongoing education about
HF, its early warning signs, dietary guidelines, medication management and
lifestyle issues.
¢
Nutrition consultations - When needed, patients may consult
with a dietician to help them adopt healthy eating habits.
¢
Medication counseling - Counseling is offered for patients who
medications are being regulated.
¢
Heart Failure rehabilitation exercise program - Patients who
are stable may participate in this monitored exercise program developed
specifically for HF patients.
¢
Monthly Heart Failure group - Patients may participate in a
special support group designed to provide emotional support for patients and
family members. This group meets the 2nd Tuesday of the month at 11:30 a.m.
at St. John's Hammons Heart Institute (inside the Women's Heart Center).
¢
Referrals to other appropriate community resources - When
needed, patients may receive referrals for social services, patient
education and other available services.
