Phases of Cardiac Rehabilitation

Orientation
Your first visit will be an orientation to the
program. One of our staff will spend approximately 1½ hours with you. We
encourage you to bring a family member or support person to this
appointment.
During this visit, we will review your
medical history and assess your current level of functioning to help us
design an individualized exercise program for you. You will be placed on a
cardiac monitor during this and all Phase II exercise sessions. You will
select a class time for your exercise and education sessions.
To
prepare for this visit
¢
Wear comfortable clothing and walking shoes.
¢
Complete and bring the self-administered history form provided in the
hospital.
¢ Bring a list of your current medications,
including name, dose, and
frequency you take each medication.
Phase I Cardiac Rehabilitation (in the hospital) candidates are patients
with a variety of cardiac diagnosis. Program components include:
¢
Supervised low level exercise
¢
Bedside education about cardiac disease process, risk factor modification
and discharge guidelines
¢
Bedside education about pacemakers, implanted defibrillators, anti-
coagulant use and stroke
¢
Assistance in setting up the appointment for Phase II Cardiac Rehab
(outpatient) at the most convenient location for the patient.
Phase II Cardiac Rehabilitation (early outpatient rehab) program components
include:
¢
Individual orientation to the program
¢
Supervised and cardiac monitored exercise sessions in a friendly and
safe atmosphere
¢
Individual and group education about a variety of topics pertinent to the
cardiac patient and family
¢
Exercise consultation
¢
Nutrition counseling
¢
Psychological counseling
¢
Stress management
¢
Physical therapy evaluation and treatment
¢
Support groups
¢
Communication of progress to the physician.
Phase III Cardiac Rehabilitation is for maintenance patients (outpatient
rehab) or is an entry program for patients with cardiac risk factors.
Program components include:
¢
Individual orientation to the program
¢
Maintenance exercise program for graduates of Phase II
¢
Beginning supervised exercise for patients with cardiac risk factors but
no history of heart disease
¢
Individual and group education about a variety of topics pertinent to the
patient and family
¢
Exercise consultation
¢
Nutrition counseling
¢
Psychological counseling
¢
Stress management
¢
Physical therapy evaluation and treatment
¢
Support groups
¢
Communication of progress to the physician.
St. John’s offers a Family Support Program that is available to the cardiac
patients’ support person. A physician referral is required. Program
components include:

Individual orientation to the program
¢
Exercise consultation
¢
The support person participates and progresses at an individualized level.
