Diabetes Education
We have a team of experienced diabetes educators that provide instruction
and guidance for diabetes self-management education and medical nutrition
therapy. Our program is nationally recognized by the American Diabetes
Association. Medicare, Medicaid and most insurance will cover our program
for patients who qualify. The nurse educator and/or dietitian can discuss
options with the patient to determine which program will best suit the
patient’s needs and insurance benefits.

Diabetes Self-Management Education (DSME)
This class in taught in a group setting to offer peer support along with
information on 10 curriculum standards. Medicare allows the 10-hour,
one-time benefit for anyone who has diabetes. After that, Medicare allows a
two-hour annual follow-up.
■
Medical Nutrition Therapy
This is provided to the individual patient by a registered dietitian.

Diabetes Support Group
Meets the third Friday of each month at 2 p.m. in
Conference Room B.
The
nurse educator and/or dietician can discuss options with the patient to
determine which program will best suit the patient’s needs and insurance
benefits.

General Outpatient Services
The outpatient services area provide a broad range of services on an
outpatient basis including:

Blood transfusions
■
IV infusion therapy
■
PICC line insertion and maintenance
■
Port access and maintenance
■
Medication injections
■
Sensory and motor nerve conduction studies
■
Foley catheter changes
■
Neuro-conduction testing
■
Pre-operative education
■
Pre- and post-operative patient care
Hours are 6
am.-3:30 p.m., Monday through Friday.
Emergent outpatient and series treatments
needed during the weekend can be performed by a nurse on the inpatient unit.
Foot & Wound Clinic
The clinic is designed to provide care to patients who are diabetic, on
anticoagulant therapy, or dealing with other health care concerns. The
clinic offers an array of foot care, wound care, patient education and a
trained staff who excels in providing quality foot and wound care. Physician referral is required for Medicare and Medicaid. Self-pay and
private insurance are also accepted.
St. John’s Clinic-Cassville
St. John’s Clinic has one of the nation’s highest patient satisfaction

family medicine, internal medicine, obstetric and pediatric care to
