ST. JOHN'S FOUNDATION YEAR IN REVIEW

St. John’s Foundation for Community Health, Springfield

Event: St. John’s Foundation for Community Health Pro-Am 2005
Date: Aug. 8, 2005
Purpose: This annual event is part of the Price Cutter Charity Championship golf tournament, which raises funds for Children’s Charities across the Ozarks. St. John’s is both a donor and recipient.
Participation: 30 teams participated in the St. John’s Foundation Pro-Am, which raised $53,080.56 to benefit St. John’s Children’s Hospital and many other children’s charities in the Ozarks.

Event: 26th Annual St. John’s Sunshine Run / Walk
Date: Oct. 8, 2005
Purpose: Provide a physical activity to encourage members in the community to get active, while raising funds for important charities such as St. John’s Regional Burn Center; Breast Cancer Foundation of the Ozarks; Springfield Public and Catholic Schools.
Participation: The 26th Annual Sunshine Run was a huge success with nearly 1,500 participants and nearly $25,000 contributed to local charities.

Event: St. John’s Diabetes Fair
Date: Jan. 7, 2006
Purpose: Increase diabetes education and awareness for individuals in the Springfield community.
Participation: The 2006 Diabetes Fair at the Battlefield Mall was a success with more than 287 participants taking part in the screenings. Activities in this event included: diabetes risk assessments; informational booths and an opportunity to talk with endocrinologists and diabetes educators to learn more about the prevention and treatment of diabetes.

Event: Valentine Run
Date: Feb. 18, 2006
Purpose: Promote a heart healthy and active lifestyle to individuals in the community. This annual event offers an opportunity for participants and spectators to see a part of Springfield that many have never experienced before.
Participation: The Valentine Run is limited to a
maximum of 400 runners and benefits patients and programs of St. John’s Hammons Heart Institute.
Title Sponsors: Hosted by Springfield Underground and presented by St. John’s Foundation for Community Health and Ozark Mountain Ridge Runners.

Event: Donna Medlin named St. John’s Foundation for Community Health Volunteer of the Year
Date: March 30, 2006
St. John’s Food and Nutrition Services director Donna Medlin was honored at the annual Mercy Service Co-Worker and Service Award banquet for her contributions and encouragement to other donors. She designed and implemented the “Food Service Education Assistance Fund,” in memory of her grandmother Josie, by seeding it with a generous contribution. She also has taken extra effort to help fundraising for the Foundation, including
serving as volunteer chair for the Sunshine Run.

Carroll Regional Health Foundation, Berryville, Ark

Event: Dogwood Canyon Health Walk
Date: Oct. 5, 2005
Purpose: To encourage a healthy lifestyle, especially among Cardiac Rehab patients
Participation: 50 participants

Event: Wreathed in Splendor Gala

Participation: Raised $13,000 for equipment and technology needs at St. John's Hospital-Berryville

Event: Carroll County Charity Golf Classic
Date: June 2, 2006
Participation: 143 golfers and $5,400 raised for People Helping People, a local non-profit that provides medical prescriptions for needy individuals. Funds also support healthcare scholarships and new technology at St. John's Hospital-Berryville

St. John’s – Lebanon Heritage Foundation

Event: All Committee Open House
Date: Sept. 27, 2005
Purpose: This event was held as an orientation / kick-off for the nearly 100 volunteers across the six giving clubs. Volunteers enjoyed appetizers, tour of the hospital, information on the foundation, information on the project, and a dedication of the grounds of the medical / surgical expansion.
Participation: Approximately 75 people attended.

Event: Golf Classic
Date: Sept. 30, 2005
Purpose: Proceeds from this event were directed to the St. John’s – Lebanon Heritage Foundation capital campaign, “Building a Heritage of Health.”
Participation: Community members / area businesses participated in a day of golf at the Lebanon Country Club. Nearly 100 players and nearly 50 volunteers enjoyed a beautiful day supporting their community hospital. Over $35,000 was raised for The St. John’s – Lebanon Heritage Foundation.

Event: IMAGINE Round the Clock Sessions
Date: Oct. 12-15, 2005
Purpose: The Round the Clock Sessions were created as a launch for the co-worker giving campaign called IMAGINE. IMAGINE stands for
“I Made A Gift In New Endeavors.”
Participation: St. John’s Hospital-Lebanon co-workers. The co-workers giving campaign committee pledged a total of $100,000 which would give them the opportunity to name a piece of equipment or a room. “Participation is Key” has been the theme for this committee and they continue every other Monday promoting IMAGINE through new co-worker orientation.

Purpose: This two-week period was set aside for Builders Club committee members as their membership drive. The Builders Club is our annual gifts program where donors are given the opportunity to give at levels ranging from $100 per year to $1000 per year for three years.

Event: The Dennis E. Hite Induction Dinner
Date: Feb. 24, 2006
Purpose: The Dennis E. Hite Honorary Society provides physicians with the opportunity to financially support the Heritage Foundation and be recognized for their contribution and efforts in making Lebanon a healthier community.
Participation: At our first annual induction dinner, longtime Lebanon physician, Dr. Dennis E. Hite was honored along with the 12 other founding members of the honorary society. $142,000 was pledged at this dinner that featured a video tribute to Hite and the founding class.

Event: Antiques Road Show
Date: Feb. 28, 2006
Purpose: This event was designed as another educational opportunity for the Louise G. Wallace Legacy Society. Road Show participants were given the opportunity to register up to five antiques for appraisal by area appraisers.
Participation: Nearly 100 guests were present for this event. A total of 200 antiques were pre-registered. The event was a complete sell-out with a waiting list of nearly 100 guests interested in another antique road show.

Event: Sister Roch Luncheon
Date: March 15, 2006
Purpose: This event was especially for Foundation board members, volunteers, and hospital leadership. During special sessions prior to the luncheon, Foundation leadership had the opportunity to ask questions, share updates, share concerns and enjoy a special time with Sister Roch and Bruce Bartoo. Following these sessions, a Spring Luncheon was hosted at the Cowan Civic Center where Sister Roch and Bartoo spoke on the importance of philanthropy in healthcare and recognized the Heritage Foundation for their work thus far.
Participation: Foundation board members, foundation volunteers, hospital leadership, and invited guests.

Event: IMAGINE Celebration “Good Day Sunshine”
Date: March 27, 2006
Purpose: This event was a special celebration for the co-workers that have pledged and are contributing to the Foundation’s capital campaign. Nearly 60 percent of the co-workers are currently giving and the group has surpassed their original goal and they continue with great passion! At this Celebration, co-workers received a recognition gift of either a scrub top or polo customized with the Heritage Foundation and IMAGINE logos.

Event: Fantasy Gala
Date: May 19, 2006
Purpose: To offer additional awareness and raise funds from the community and gala guests.
Participation: This event featured many levels of participation from a private reception with Peter Cetera at the home of Craig and Tracy Curry, a visit to St. John’s Hospital – Lebanon with special opportunities for the co-workers to meet Cetera, social hour, dinner, concert, and post-party all rounded out this highly-acclaimed second annual event. Nearly 100 guests attended the private reception on May 18. 595 guests attended FANTASY GALA 2006.

Event: IMAGINE Celebration “Strawberry Fields Forever”

Purpose: Co-workers that are participating in IMAGINE are regularly recognized
Participation: IMAGINE members enjoyed a strawberry shortcake party and had the opportunity to wear their IMAGINE polo or scrub top. This event was designed especially to thank co-workers for their participation and offer others the opportunity to participate.

Event: Auxiliary Tea Party
Date: June 28, 2006
Purpose: The St. John’s Hospital Auxiliary is a vital part of the hospital family. The Auxiliary has made a generous pledge to the Foundation of $60,000 over the three year campaign. To recognition each member of the auxiliary was our goal. Auxilians will enjoy a special time together as they enjoy a tea party at the hospital. Heritage Foundation leadership will award each auxilian with a Heritage Foundation commemorative tea cup.

Event: St. John’s Hospital – Cassville Community Picnic
Date: Aug. 10, 2005
Purpose: Fun event open to co-workers, doctors and the public to raise funds for the proposed Rehab/Wellness center at Cassville hospital.
